Using the drop-down menu below, select the semester you are petitioning for. Add each course, including the CRN, Prefix, and Number. This information can be obtained from your registration history in Self-Service. Check the appropriate box next to the action you want the committee to consider. If you need to add additional courses to your petition, select “Add Another Course.”
To access your registration history, please visit https://my.okstate.edu and click the Self-Service icon. Select Student Profile and then choose the term (semester) from the drop-down menu in the upper left corner. The list of registered courses will be displayed. You will need to include the CRN, the Course Prefix and Course Number listed in the course information boxes below.
Your written statement should explain to the review committee what extraordinary circumstances occurred during the semester you are petitioning for. Address why you were unable to drop/withdraw by the published deadlines. If you are not including all your courses from the semester on your petition, explain why the extraordinary circumstances only affected the course(s) listed. If you did not attend the course(s), include a letter or email from each instructor verifying that they have no evidence suggesting that you attended or participated in the course. The documentation included should clearly indicate the impact the extraordinary circumstances had on your course(s). Failure to include a written statement and documentation will delay committee review.
