Any event as identified within the Fraternity & Sorority Affairs Event Policy within the
Fraternity & Sorority Affairs Policy Handbook
; must be registered. Please review the following guidelines before completing the form:
All chapter events (with our without alcohol present) must complete this corresponding online form in its entirety by the appropriate deadline:
Events with alcohol- Submit a minimum of 7 days in advance.
Events without alcohol- Submit a minimum of 2 days in advance.
Any event registration form that is submitted late will result in an assessed late fee to the chapter of $75 per day.
Chapters that fail to register an event with alcohol and host the event will result in an automatic $500 fine and referral to their respective Council Judicial Board.
If more than one chapter is hosting the event, all chapters are responsible for also registering the event. NOTE: No more than 3 chapters can co-host an event together.
If alcohol is present at the event, all organizations must follow all guidelines set forth within the
Student Organization Alcohol Policy
, inter/national policy, state, local, and federal law.
Before Proceeding:
Make sure you have all the following information to complete the form:
Event details including location, address, estimated number of guests, start/end time.
Name and Contact Info of Event Coordinator, Chapter President, Chapter Advisor, and Risk Manager.
If alcohol is present: How served, Name & Ages of Sober Monitors, Designated Driver Program, Copy of Liquor/License and Insurance of Third Party Vendor.
